Vendor Approval Application

To render services at Arlington Acres, your business must obtain pre-authorization and agree to the terms of service.

 

Please fill out the form below and send us the necessary documents. We’ll notify you via email once you’re approved!

Requirements

  1. All Vendors and/or individuals providing a service at Arlington Acres shall carry and maintain in full force and effect the following: 

  2. General liability insurance at $1,000,000 per occurrence / $2,000,000 general aggregate AND statutory minimum employers liability, where required by the state of Ohio. 

  3. Drone users must also carry Unmanned Aircraft Insurance and pre-register with the FAA to fly in the airspace at Arlington Acres due to the proximity to the airport.

  4. General Liability Insurance carrier shall name Arlington Acres Ohio, LLC AND Eugene, LLC. as additional insured.

  5. It is the responsibility of the Provider to maintain the required coverage and send updated COI's upon renewals to arlingtonacresoh@gmail.com

  6. Caterers should be licensed and certified to serve food.
